XI. PROCEDURAL CHECKLIST

When to do

Activity/completed by/distributed
Semester before Student attends information session conducted by FE Coordinator.
Site identified by student and Faculty Advisor. Site Intention Statement competed and signed by Faculty Advisor.
FE Coordinator regesters student for INLS 299, based on Site Intention Statement.
Field Experience Agreement (FEA) completed by student and site, approved by FAculty Supervisor and put in FE Coordinator's mailbox.
DEADLINE for submission of FEA is one week prior to beginning the field experience work.
At start of FE Upon receipt of FEA, information packet sent to Site Supervisor by the FE Coordinator.
20 work hours after start of FE Progress report completed by student. Copies to Site Supervisor and Faculty Supervisor. Any goal modifications to be discussed with and approved by Faculty Supervisor.
60 work hours after start of FE Progress report completed by student. Copies to Site Supervisor and Faculty Supervisor. Any goal modifications to be discussed with and approved by Faculty Supervisor.
During FE Faculty Supervisor visits student and Site Supervisor at site or conducts telephone interview.
Seminar meetings (3 consecutive) attended by student. Attendance required.
End of FE Completed paper submitted to Faculty Supervisor.
Site evaluation form completed by the Site Supervisor, reviewed by the student, signed by the Faculty Supervisor and given to the FE Coordinator.
Student evaluation form completed by student, submitted to FE Coordinator's mailbox.
