Your email address is asked for on this survey. You may of course choose not to provide this information. We ask for this information only so that we can follow up with you if we have any questions about your responses to this survey.
If you do provide your email address, we will not use it in our data analysis. All data will be summarized – that is, data from multiple individuals will be combined so that only collective analyses will be possible.
We will not use your email address or any information that may be personally identifying in any of the reports from this study.
At the conclusion of this study, two $25 gift certificates to Amazon.com will be given away to two individuals who provide their email address (if you don't, we have no way to get in touch with you!). These two individuals will be selected at random from everyone whose email address we have.
It is your decision whether or not you want to participate in this study. If you decide not to participate in this study, you will still be entitled to use the virtual reference service and all other services offered by the State Library of North Carolina.
